eHealthcare Leadership Awards Entry Form

Please note the following before completing the entry form:

* Complete all information and check that spelling is correct. Enter the name of the Web site organization exactly as you wish it to appear on the certificate should an award be received.

* Be sure that you select just one Group Classification per entry form. You may enter as many Award Categories as you wish (up to the maximum of 12).

* Complete an entry form for each Web site or URL you wish to submit. When you have submitted an entry form, use the back button on your browser to go back to the entry form; make changes and then complete the submission process.

* As appropriate to certain award entries, forward to us other information that can't be readily accessed on your Web site and that you believe is critical to the evaluation of your entry.

* If a Web site requires a password to access most content/functionality, please indicate this in the appropriate box on the form. You will be contacted later to supply password and access time for judging.

* Business/process improvement applications may be requested to supply additional support so that the site can be fully evaluated.

* Complete all information and check that spelling is correct. Enter the name of the Web site organization exactly as you wish it to appear on the certificate should an award be received.

* All entries are final.
